Does the cumulative stafford amount for medical students include any debt incurred from undergrad? Say I had $30,000 from undergrad. Would it be MAX = $138,500 - $30,000 = $108,500?

Yes, Stafford loans are cumulative. But for medical students, the aggregate maximum is $189,125.

What if the undergrad loans are fully paid off? Does the undergrad loan amount still get deducted from the aggregate maximum?

No, the maximum includes loans outstanding.
