cv for recommenders

I used the search function, but did not specifically find the answer I need.

I will be applying in 2013. I am considering asking one of my present professors for a LOR. I know that I need at least a transcript and a cv to give to them, but my question is, how should the cv be organized and what should it include? The things I find on the internet are mostly for business or for research position employment.

I'm thinking, aside from personal information and educational info (major/ GPA, university), should I then include awards, then EC's in reverse chronological order and then employment history? Lastly hobbies, sports, etc?

I understand that some schools don't like LOR's that are over a year old. If I get this in May, would it still be too early or should it be ok? Anything else I should include for my recommenders? Thanks for any advice.
 
My CV was something like educational info, then employment/volunteering, misc EC's, international experiences, then awards. I agree it's best to tack hobbies onto the end if you want to include them. A business/job resume usually requires some sort of objective/mission statement and skills section, neither of which are necessary in this case.

Are you applying for matriculation in 2013, or are you actually applying in 2013? If it's the latter, your LOR will be pretty damn old by the time you apply. If you're going to get an LOR even a year early, you might want to ask your writer to save a copy so that he/she can update it when you apply.
 
Thanks for your response, rhesuspieces. I am applying in June 2012 for matriculation in 2013, so the LOR would be just over a year old if I am able to get it in May. Is that really too old? I am afraid to wait till later as it is a class of almost 300 and she is likely not to remember me at all later on. I have been going to office hours almost weekly and at least she recognizes me and knows I try hard, am interested and am acing her class so I don't want to miss the opportunity. Next fall my classes will all still be very large, so the prospects are slim like always. This is the frustrating part of going to a large public school.
