DEA application: which one? practitioner/teaching inst/hospital

Currently a 2nd year resident. Planning to apply for my DEA license. I've heard rumors that you should pay for ur DEA fee instead of getting it waived through your residency program if you want to moonlight outside of the program. If so, do I select practitioner, teaching hospital, or hospital/clinic? thanks.

You are a practioner on the application. And while you do need your own, independent DEA number to do moonlighting, it works everywhere in the state, not just at your residency. Just because they are paying for it, doesn't mean you can't use it else where. Unless you already have the moonlighting lined up, you will need to have a business, i.e residency, address for the them to send the license to; they will not send it to your home address. The only exception would be if you're planning to moonlight in another state. You need a seperate certificate (and get to pay for them all) for each state you work in, even though the number doesn't change. If that's the case, the place you work for may pay for it.

what's the question about 'Check here if you require order forms. (Schedule I and II only)' Order Forms Checkbox: Select ONLY if you intend to purchase or transfer Schedule I or II substances. Order form books will be issued after approval of your DEA registration.

That's if you intend to have Schedule I and II meds in your office and dispense them out of your office or use them in clinical practice. It has nothing to do w/ writing scripts for people to pick up at the pharmacy.
 
I was always told one had to get one's own DEA number to do external moonlighting. So that is what I did. And nobody paid for it for me. I don't think our residency program pays for residents or fellows to get a DEA number. You have to pay for it yourself. You can us the institutional one to write for narcs for your inpatients and resident clinic patients.
