Dean's letter from dental school

Im a BDS graduate from India planning to do my PG in USA, they asked for Dean's letter, can anyone send me a sample letter to know what it should be like, its not easy for me to get one though. Its being 5years since im graduated and I know our dean is changed. But still if I can have a sample letter i might be able to communicate with the new dean and ask him for one.

Has to state your Class rank, your GPA, if your school gives you one, and how well you performed in school
