Something along the lines of:

Dear Admissions Committee at X University,

I want to thank you for offering me an acceptance to the dental program. However, at this time I have decided to commit to a different University. I would like to withdraw my application and vacate my seat. Thank you.

Dear...

It is with great regret to inform you that I will be declining the offer of admission for the entering class of 2018. Thank you for believing in me, and for offering me the opportunity, and I am sorry that I cannot accept the offer.
