delayed PTAL and losing spot

Last year I matched into a program in California but unfortunately was unable to get the PTAL in time and had to give up the position at the hospital. Now I am applying again this year and I have a question. My program director wrote a letter explaining what happened as well as some positive points about me. I am not sure how to approach this when applying again. Do I add this letter as one of my four LOR on ERAS? Or should I just bring up the letter when I go to the interview?

Just curious but what was the delay?
 
The board lost my L6 forms that I submitted before January. After January the new law required that the forms be submitted from the hospitals directly so when I was notified on April that they lost my forms I had limited time to get all the L6 forms.

April should have been enough time to get ptal stuff from hospitals. sounds like something not telling us.

PTAL takes months after getting a completed application. The hospitals sent out all my forms by end of may but this wasnt enough time to get my PTAL by JULY
