I don't have a strong sense of what drives you as a physician. I don't think you have a sufficient demonstration of service orientation according to the AAMC definition. The tutoring goes under "teaching". No offense about caring for foster kittens, but not all faculty will see this as fulfilling the service orientation box. That leaves you with a recent activity with food bank/distribution for around 100 hours, which will be below the threshold for many schools unless there is more on your application that you have not disclosed from your undergraduate days.

I agree if you get Ohio residency, your options open up. The challenge is whether you really understand the issues Ohio citizens face, and how have you gotten to learn more about these issues that could be a bit different from your experience in California. Find out the rules first. Pay close attention to mission fit.

I also presume the 512 MCAT came before you started your SMP?

For starters:

I don't think the issues with restricting abortion/reproductive health or vaccinations are a huge concern in California. Ohio has had big problems with opioid addiction.

Do not retake the MCAT. Spend more time with non-clinical volunteering. Your hours at the food bank should be around 200+. You can drop the ED volunteering position if you need more time since you already have ample clinical experience with your past full-time job.

If you keep your CA residence, the only in-state MD schools I would advise applying to there are UCD, UCR (only if you're from the IE), and CUSM.

CHSU COM is too new of a DO school. Consider Touro NV instead as they are close to Southern California.

Thanks for the advice. I figured it would be wise to stay involved clinically since my past clinical experience was over a year ago (I quit my clinical jobs to start the SMP). I can drop the ED position, but my food bank only has one four hour shift once a week. Doing the math, this gives me ~130 hrs. Should I look into another non-clinical activity to supplement the food bank hours?
Yes, look into your local homeless shelter, soup kitchen, or similar areas.

If you are in Cleveland, there are plenty of opportunities: Food Bank, Meals on Wheels, City Mission, Dress for Success, LGBTQ center, Habitat, Home Resource Center, etc.
