I am very interested in Dictation and was wondering what type of dictation solutions people were using and how they were working. I have seen some recorder solutions where then you send the files to the transcriptionist, but that seems a bit clunky.

Any information on this topic would be helpful.

Look up dragon dictation (aka nuance)

It works, but needs some 'teaching'. Are you a physician? The medical version of it recognizes medical jargon but costs >1500 dollars last time I checked. The general purpose one ran 150, I think.
 
Dragon medical 10.5 at my institution. Not perfect but certainly faster than typing (and I'm a good typist). Have created numerous templates and find it very useful on inpatient rotations where we can't use the phone dictation/transcription.
 
If you have an iPhone 4S (or jailbroken iOS5) you should seriously look at the following...

Remote Dictate (currently free in app store and only one that currently works on PC)
Air Dictate (MAC, but PC verison coming?)
Vocal (MAC)

these will allow you to use Siri on iPhone 4S to dictate text into your computer without typing - voice transcription. It is like using Dragon Dictation but through your iP4S and without the need to spend $$$ on the Dragon software.

definitely worth looking into.
