Did you send a Photo form w/Columbia Secondary?

So I just submitted my columbia secondary a second ago and afterwards it said the usual "please print proof of payment"... blah blah blah.. but then it said if you paid by credit card, to send in a passport size photo with Columbia's "photo form" to the below address.

I couldn't find the photo form anywhere. Can someone please help me out here? 😕

I don't think there was a form. I cannot remember too well since I sent that stuff in at the end of June. I mailed a check to them so I think I just included the photo in the envelope.

yea, i was confused.
one one part of the app it says like extra forms or something, but there wasn't anything there to print out except for an instruction booklet.
i'm not sure if there's a photo form or anything on there.

Didnt the columbia secondary say to forward a photo with your committee letter packet? Is this not necessary?

I believe they do require a photo. Since I was on the med campus, I just stopped by the admissions office and handed it to them. I don't think it will be a big deal if you just mail it to them in an envelope with your AMCAS ID # and everything.

I think someone called the admissions office to ask about the photo form, and they said to just send it in the photo. theres no form. make sure to put your name and ss# or amcas# on the back
