Is there a difference?
How much more selective are they compared to regular med schools?

Do they really pay your tuition AND give you 25K on top of that? (if true, that's insane)

MSTP programs are a subset of MD/PhD programs. All MSTPs are federally funded through the NIGMS (part of the NIH). Because of this guarantee, they are generally more selective. Yes they pay all tuition and give you an ~ 25k stipend. Some non-MSTP MD/PhD programs are funded as well, but some are not. It varies.

Ah I see.

So that explains how an applicant with a perfect resume can get rejected by MSTP... because MSTP is more selective than MD/PhD, which is more selective than med schools.

Now it makes sense.
