I keep trying to request an activation code but it requires an OSU usename. I don't have an OSU username. I can't find on their site how to create an OSU username. I cllicked "Forgot username" just in case I was supposed to have already had one, but it says I'm not in their system.

This is to complete the secondary app btw.

You should have received an email from OSU that contains a username and an id number that are both unique to you. This email also had a help number in it that you could call. Do you not recall receiving an email such as this?

I received an email like this from OSU yesterday (3rd batch), completed it immediately so that my username and password had been established. Just earlier this morning now I finally received a link to the supplemental and I had no troubles getting to it.

Was your email solely a link to the supplemental application or did it also have a link to the 2 other forms you have to fill out? I got an email saying i needed to complete the supp app, and the 2 forms, but the supplemental app didn't have a link (the 2 forms did have a link though)

The email I received from OSU yesterday did not contain an ID nor a username. Here it is copied and pasted:

"We have received your Autumn 2013 AADSAS application information. To apply to The Ohio State University College of Dentistry, please access Ohio State’s self-service website to complete a supplemental application.

You will need an Ohio State Username and password to access the supplemental application. For many new applicants, you may not yet have or you may not know your username and password. To establish your Ohio State Username, please visit my.osu.edu and request an activation code. If you do not know or have forgotten your Ohio State Username, click on the ‘Forgot username?’ link below the login box in the Current Student section. If you have problems accessing your information, please call 614-688-HELP (4357).

After you have your Ohio State Username and password, please complete the supplemental application. An application fee is assessed upon submission of your supplemental application to the College of Dentistry. The application fee for domestic applicants is $60; for international, $70. You may use Visa, MasterCard, Discover, or American Express to pay the fee.

Please be patient as it may take several days for your online supplemental application to be received by the College of Dentistry Office of Admissions. Once your supplemental application has been received, your application will undergo phase two of its review.

If you have not already submitted your Dental Office Observation Form documenting the minimum requirement of 20 observation hours, please visit the following web page: http://www.dent.osu.edu/Admission/observationform.php

In addition, the Non-academic Experiences Form must be submitted before you are eligible for an interview invitation. The form is available at the following web page: http://www.dent.osu.edu/Admission/nonacademicform.php

Sincerely,

Office of Graduate and Professional Admissions, on behalf of:

The Office of Admissions

Guess I'll have to call the number then? Ugh.

Weird. It seems that either you missed an email from OSU a few days ago, or they didn't email you the first emails that I got. Possible help number listed below. I haven't had a problem so I'm not sure what to tell you guys, but at least you can take a look.

Receipt of AADSAS Application (7/13)
- Includes what to submit, important notes, what to do, and what not to do.

Ohio State University Username Activation (from the OSU IT Desk) (7/19)
- Tells me that to activate my username I need an activation code (link). Continues and tells me specifically what my username is as well as my unique ID. For assistance, call (614) 688-HELP (4357).
- Shortly after this I received my activation code and proceeded to activate my username without any problems.

Your recent application to the university (Today)
- Exactly as faithbuild describes it.

To OP: I would check your inbox, trashcan, and all junk-related folders to be sure that you didn't skip any emails from OSU. If the problem persists, call the help desk or someone at OSU. You need the username which is (was) given to you prior to the supplemental email being sent out. Sorry that this is all the help I can really offer. I just thought it went as well for everyone else as it did for me. Good luck and let us know what you find.

Thanks for the help and concern! I contacted their ITS and they emailed me my username. I'm not quite sure why I never received the first email. Wasn't anywhere in my inbov/spam/trash/etc. Everything's good now though, thanks!

I didn't receive an email address either, but luckily the mail app on my app made it easy for me to search back to when I applied to OSU for undergrad. Turns out I had to use the same username as before.
