Direct primary care, and it’s off shoot direct specialty care have become really popular and many more physician friends I personally know have gone that route. Wondering if there was a market for DSC with PM&R and what that would potentially look like. I know rheumatologists who have been able to deliver personalized care while maintaining nice work life balance with comfortable compensation. Welcome any ideas here thanks

Possibly, but most likely in the orthopedic/MSK area of PM&R.
 
This is what I want to do with my life. I'm not sure it'll be financially lucrative on it's own, unless you somehow carve out a niche doing home visits for rich disabled people. I'm planning to supplement with a small traditional inpatient practice. I disagree that there would be a major market for MSK-focused DSC, unless by this we mean a cash-only place for PRP wherever you want it injected or whatever.

What other part of PM&R do you think would lend itself to a concierge type business?
 
After a 10 year practice doing sports and spine, I'm 9 months into a DSC MSK practice:)

Can you explain your set up briefly?
Solo micropractice. I tend to see the "mystery" cases that no one seems to be able to figure out. I perform diagnostic ultrasound, EMG, image guided injections (hydrodissection, steroid, HA, etc.). Not sure if I want to restart regenerative practice again. I have stopped doing interventional spine procedures but may rent a C-arm locally to do them every now and again. I have a flat rate for new patient and follow up appointments.

My new patient evaluations typically are 60-90 minutes and follow ups are about 30-45 minutes.

I'll see at the end of the year.
That being said, I haven't had to change my spending habits really and my expenses per month haven't changed.
Big ticket items for me right now are mortgage and two cars.
We have about $12-14K per month in expenses (include 529s, mutual fund investments in this number) and we have been able to cover it and have stayed in the black for the past 9 months.
The biggest thing I could recommend is pay off your student loans as soon as possible. I became a lot more comfortable taking risk when this was gone.

More importantly, I'm happy and practicing exactly how I want and spending maybe too much time with the family...haha!
