Does this sound like a reasonable model:
Discrimination model: schemas --> perceptive sets --> stereotypes --> belief perseverance --> prejudice --> discrimination --> racism
Anything to add or change?

Also, am I understanding the differences between stereotype, prejudice, and discrimination correct?
stereotype = general THOUGHT about a group of people
prejudice = a JUDGMENT about the group
discrimination = an ACTION against the group based on the prejudice
racism = an ATTITUDE against the group
 
Stereotype = general belief about a group of people
Prejudice = a preconceived, baseless judgment about the group that affects one's interactions with members of that group
Discrimination = as you described above
Racism = justifying prejudice and discrimination through the belief that certain traits are inherent to that group of people (i.e. believing the stereotypes to be biological fact vs. opinion)
