This is probably a no brainer but just wondering what your clinics' protocol is for disposing of unused opiates in someone who you are rotating or changing the script. At my previous practice the hospital pharmacist did all this? Who collects the meds? How many people need to document? And most importantly, what do you do with the med?
 
We have a double signature log.

Name, date, med, dose, qty, sig 1 sig 2.

Meds collected by LPN, RN, MD and counted by 2 staff.

Mixed with bleach or alcohol and hot water and made into a slurry.

Dumped into sharps container from there.
 
OxyContin, alcohol, hot water, and slurry sounds like an opiophile recipe to beat time release!

Plus the alcohol sterilizes the sharps so prolly safe to drink.

Reach in and grab them.

Once the sharp container is full it is sealed and bagged and sealed again. Then picked up for incineration.
