So I am currently stuck thinking about what I should write about.

I could write about:

1. Immigration, struggling here in America but being able to make it out and move overseas for high school with my family in a very diverse country and learn from people around me there and how I've come back here for college to give back to those communities in clinical and non-clinical settings.
2. The unique clinical experience of providing care for autistic children and how it has shaped my perspective on working with different people and especially those who don't fit a certain "box".
3. How I struggled with my identity as a Muslim in the US and how I hope to use my education to break down stereotypes and false narratives.
4. Resell sneakers and how I started my own organization at my college to share that passion with others and how it is helped me connect with so many different people in my city

I have already mentioned my work with underserved communities in my PS but I am wondering if I could take a different angle? Should I combine a couple of these together?
 
1-3 have a lot of potential as topics for a diversity essay. They could also work as response to a challenge overcome essay. I don't quite see #4 will be as effective as the other three in responding to a diversity essay. However, reselling sneakers should definitely be in your app, perhaps as an activity or in response to question about initiative or creativity or perhaps just a tell-me-something-else-about-you-outside-of-medicine question.

I don't know the specific prompt you're asking about or the word limit, but in general, pick one of your topics and tell that story well in response to the prompt. Don't try to cram all 3 examples into one essay.

The immigrant experience is always good

Admirable, but nope

Plenty of med students are Muslims these days

That's different!

No, because you're supposed to do that.

The prompt is not what you bring to Medicine, but to the Class.
 
The prompt is :

"The Admissions Committee encourages you to share life experiences that you may have had and/or important factors in your background that illustrate your readiness for practicing medicine in a multicultural society."

Thanks. You can't go wrong with the immigrant story (life experience + important factors). The other suggestions don't really work with the prompt; I suspect you can fold in your Muslim identity with your immigrant backstory. This is really a variation of Tell Me About Yourself with the prompt guiding you on your focus.
