Do I need an English name for my US clinical rotation?

I found it is difficult for native English speakers to pronounce my name correctly, even if it is spelled in English alphabets. On most occasions, they do not know how to pronounce it when they look at my name on my passport (which is my real name). Now I will go to the US for clinical rotation, do you think I should have an English name?
However, I'm afraid that I will have to use the name on my passport for residency application. So I fear that if I use an English name during audition rotations, American faculties and students will not associate my real name with me during my residency application.

I don't think you have to change your name. However, if you feel strongly about it, you can introduce yourself as "Hi my name is Firstname but I go by Englishname. On your application, write your name as Firstname "Englishname" Lastname.
