Do medical schools only see the grades on the transcript?

Varies depending on the university. Best thing to do is to get a direct official copy from yours.

Well hold on here. If you drop a course on the first week, at 99% of schools, there will be no record of this on your transcript. They will not see this.

This is different than an official "withdraw" in which a W is recorded for a class. They will definitely see this.

Tldr: if you drop a class and it's before your official "add drop deadline" or whatever system your school implements, so there is no record on your transcript, they won't see it.

Ok because my university says:

02/05/2018
A class dropped on or before this date will be retained on your academic record with a status of dropped.

But it will not be shown on transcript as "W". Medical schools will not see any of this then correct?

So it's retained on your academic record but not recorded on the transcript at all?
Or will it be on the transcript? The transcript is the only thing that matters here.

It will be retained on academic record, but not on transcript.

As it's been said, most schools have an "add/drop" period where there would be no record of dropping a class (usually 1-3 weeks). After that, it's a W (and at some point an F if it's too late)

It will be retained on academic record, but not on transcript.

At one of my undergrad schools, the academic record was only used in-house to record the total number of drops so that students were kept under the drop limit. Pretty sure this info isn't shared with med schools.
