Do residency programs or licensing boards look at your full school record?

I think this is too vague to answer, not sure if law2 is reading but maybe he would say err on the side of transparency w/ licensing.
I had an incident outside of school that resulted in a professionalism inquiry/investigation... no formal institutional action resulted (no probation, suspension, etc) and I stand by my innocence in this issue! It's not drug related or anything like that. The issue will not appear on a dean's letter (It legally cannot). However, I think there is a disciplinary record of this with the school.

Are there any future applications that I will need to disclose this on? In other words, will there be anything relating to ERAS, specific residency apps, or state licensures that will pull my disciplinary record with the school?

Yeah I would. This stuff can come up, either through a broadly worded question about ever being a party to a disciplinary action/proceeding, or through back channels when a PD is talking to a dean or faculty. If you were open and honest in your application/licensing forms, this probably doesn't hurt you. If it comes out that you intentionally omitted this (ie lied about it), that can always hurt you worse. I would probably downplay it as much as reasonable but if there's a question on any form that arguably covers this, make sure it gets mentioned so you can never be accused of hiding it/lying about it.

2. It very well could come up in licensing applications, some will ask specific questions about professionalism and disciplinary actions and you need to read every question very carefully.

I asked today actually - it's not an official action and it's not on my academic record. An advisor told me a few weeks ago that she looked in my record and it was completely empty but I didn't understand what she meant by that because it was under a different context... They told me that the school keeps a seperate file for internal use and they said there would be a record in that file for a while but no official action was taken nor would the school disclose that file to another school/program. I'm glad to hear this isn't the USSR where you're guilty before proven innocent lol.

Now - I'm going to call up the licensing board from my state on Monday and they can tell me if anything from that file can come up when they inquire to the school about my time there. I've just heard horror stories on here about people not disclosing things in full for whatever reason. I'd rather err on the conservative side but I don't want to give out information that isn't relevant either! Heck, if you have a skeleton in a locked closet and nobody has the key, why give them your key? 🙂

It doesn't matter if it's in the record or not. People know about it. They can speak. If a residency program director know someone on faculty and they talk about you, this could come out. Doesn't matter if it's in your permanent record, or if there's no written record of it. Loose lips sink ships. If asked about it on an app, you had better own up, because you can get caught even if there isn't written evidence.
