Doctor's Notes/ Medical Records?

So my recruiters last week told me that I need to get a note from my PCP saying that none of my medical history will be an issue in the Army.

But will I need to get notes from ALL my previous doctor's visits? (ER to get 4 stitches on a cut, throat infection, bronchitis, allergy test from 2008)

The recruiters said that I probably won't need ER records but I just want to make sure and cover all my bases.

Will the note from PCP suffice or will I need to go back to my childhood and collect a note on everything?

Unless you are still being treated for something like bronchitis or allergies there is no need to mention the visits. If there is a scar of some sort, you will probably need a doctor's note explaining that there is no residual effects from the injury/surgery meaning that there is no loss of motion or sensation.

Just try to stick to what your recruiters say to the T.
