Documenting volunteer/shadowing hours

So I'm a sophomore in college and I'm getting a little late start on my volunteering/shadowing hours done. What is the best way you guys have found on documenting these hours that you guys spend volunteering and shadowing?

Another thing...

How does the interviewer know that these hours are legit anyway?

Should I get signatures on everything?

The only documentation needed is a letter of evaluation from a dentist you've shadowed if you choose to submit one (some schools require, some recommend, some don't care). Otherwise, on the AADSAS, you only list the activity, a 175-char description, hours and the dates.

The interviewer (if open file) has no way of knowing but if asked about it, you would have to talk about it.
 
None of my interviewers have expressed the slightest bit of doubt about any of the activities I included on my application, and my hours are quite high in lots of different things. Don't worry about it. Just be truthful, obviously, and have good experiences to talk about in case you're asked about any specific activity--especially those with which you were most involved. Also, every school I applied to required a dentist's recommendation, so I would absolutely plan on securing one.
 
I have been documenting my shadow visits for over two and half years. The method I started with was writing the date of the visit at the top and then the total number of hours i spent shadowing that day. I would then write a short entry about who i met that day and what I learned. All of my entries have been handwritten and are no longer than one page each and no shorter than half a page. Recently, I have added a little more detail and included the time which I shadowed (ex. 0900 - 1200 hrs ). All this is done using plain lined paper. No need to get fancy. Just make sure you keep your journal in a safe place. If anyone has any ideas about what else to include, I would be very interested!
