Can’t you check the unofficial transcript and see what it looks like? It should have the same info as the official. Or is this hypothetical and you’re debating taking a pre-req online?
To answer your question, OP, no, it does not. I took a course online last semester, and I'm looking at my (unofficial) transcript right now, and it doesn't say anything.
That being said, keep in mind what gonnif said. If you are prompted to clarify if something is online, then you should by all means tell the truth. I'm not really sure why you're asking, but I don't think taking a class online would necessarily look bad.

I haven't done classes there yet, I've done classes at a different college. I was checking mainly because I was debating on doing some prerequisites online*
(I'm not sure if blended counts either. I have half my credits online and the other in person..
Just because I honestly don't remember. Does AMCAS even ask if a course is online? I'm pretty sure they just check themselves through the NSCH. I think the only questions are whether the course was lecture, lecture+lab, or lab only, as well as if it was a special course type (None of the options to that question is online).

At the end of the day, it does not matter how you got the knowledge...just that you know the material and can apply it to new situations (a la MCAT and real life). That being said, it is best to contact the registrar and ask directly. They will know the answer and there is nothing to be ashamed of or afraid of.
If you need a class such as general chemistry or physics, please contact an instructor such as myself.
