Sometimes you only have to observe what the majority are doing and run the other way to find success. Dr. M, that's what you did and it's awesome.

Too often people look for traditional methods.

There have always been many different niche in retail pharmacy. While I pursued pharmacy managment, I used to own a DME co. with respiratory and closed door pharmacy.

But one niche that's never been tapped is the 340B market. 340B is a federal program designed to support nonprofit health systems by providing many key outpatient drugs at approximately 30% below cost. These facilities can either own or designate a retail pharmacy to service their employees and residents of the county. Do your own google search to find out more.

In a nutshell, it's an opportunity to increase your gross margin dramatically.

To give you an example, Protonix 40mg tablet 340B price is 20 cents.

So, Dr. M, you and your partner should go find you a local 340B hospital and be their desinated retail pharmacy before someone else does.

Of course, typical 340B hospitals are in poor areas.

Some enterprising pharmacists will profit handsomely by tapping into this program while servicing needy patients.

Gloom and Doom on this board is comical. Also comical is hatred of retail chain pharmacy. Life is short. In fact, it's too short to live one while doing something you hate.

Good to have you back! Actually, we have tried to service two 340B programs: The Ryan White Foundation and the Pinellas County Health Plan...Neither would accept us as they have a Pool they draw from...We will try again this year. Hopefully they allow us to service them!

I hear the pay sucks.

You´re assuming I am receiving a paycheck! :laugh: But that will get fixed with time. You have to understand that out here, you can´t get things done fast.

Things are going good for me. In the three months that I have been out here, I´ve done many things that I would never thought that I would do in my life time...and I love it. Conditions are hard but manageable. At the location I am at, we have hard shelters to sleep in, two hot meals a day and relatively safe. Much better that what some others get. When I get back in January, I will right a detailed post on what I´ve done out here.

And before I forget, clinical pharmacy here in a deployed environment is very similar to what we do in the United States at Walgreens, Hospitals and Schools of Pharmacy. It is what you make of it. If you decide to not get involved, then it won´t be clinical. If you decide to get involved, not only can you make a differnece in your own life but you can make a huge difference in the lives of the patients you see on a daily basis.
