Dear Pharmacists and Student Pharmacists,
The second-fastest developing drug issue in the nation is prescription drug use.😱 As pharmacists and future pharmacists, we are in a position to directly impact the outcome of this important issue.
WE NEED YOUR HELP!
We are fourth-year student pharmacists at the University of California, San Francisco.
We are conducting a survey designed to evaluate the use of the Controlled Substance Utilization Review and Evaluation System (CURES) Patient Activity Report (PAR). The purpose of this survey is to evaluate pharmacists knowledge of and access to CURES PAR, assess the usage rate of CURES among pharmacists and also to assess participants readiness/willingness to utilize the prescription drug-monitoring program.
This survey is anonymous. Participants personal identifiers will not be distributed to anyone or associated with the answers that are provided. Personal identifiers are used solely to determine winners of the raffle prizes. This survey is voluntary and participants may exit the survey at any time.
The survey will take approximately 10-15 minutes to complete. After completing the survey, you will be provided with a link to enter a drawing for one of four raffle prizes🙂:

Apple iPod shuffle (retail value of $49)
One of two Starbucks gifts certificates redeemable for $25 dollars
