Dumb formatting question for scholarship essay

Hey, writing an essay for a scholarship. Have a few questions.

1. Should it be double-spaced?
2. Should I put my name, date, and AMCAS ID as a header or just as a single heading on the first page?
3. Does it need page numbers? [only 2 pages if I double space']
4. Does it need a title?
