I listed a contact for a volunteer program I did after freshman year using the e-mail that I had been in contact with way back then. It turns out that she since got married and her e-mail changed to include her new name.. I am worried med schools with get an error message if trying to follow up on my hours and think I lied. Should I send schools an update??

No. They will email to ask you for updated information if they need it.

I finished undergrad close to a decade before applying to med school, half the program directors don't even work for the university anymore. Provide the best info you have and if they want to follow up they will. But it has been my experience that unless you say you did 300 hours in a month while also taking classes, they generally accept the data without issue.
