So your clinical exposure is 1.5 years doing secretarial-type work for someone who happens to have MS (I mistakenly thought "personal assistant" implied feeding/dressing/bathing), 20 hours in a free clinic, and a yet-to-be-started peds ICU gig that will last 5 months. This is going to look pretty thin on your application.
Were any of your many, non-specified jobs even remotely related to working with sick people?
Without more clinical experience, adcomms will wonder about your motivation for becoming a physician. Maybe MD/PhD programs don't care about this so much, when one has very strong experiences doing research, as you do.
