I have this same question. I have a very meaningful volunteer experience from 16 years ago that I want to include, but I don't have a name or contact number for that person anymore. This volunteer project was through the family medicine department at a local medical school and I wonder if I could contact the current project manager to see if I could list them as the contact person.
AMCAS allows you to use yourself as a reference in cases like this. I had to do it twice on my application as there was no way to track former supervisors from over a decade ago who'd moved on.
