Elective in U.S. or not?

Hello.

I'm a foreign medical student at German university (4. year) and would like to ask, if it's possible to do some electives in one the med. schools in u.s.. Do i need tto speak fluent English? should i pass the TOEFL first and then try to start some of the electives there?
