Is it possible to do an IM "fellowship" or just some additional training in EM after say an IM residency? I've heard of people going back for a "second" residency, but could you possibly get extra training? I also wonder if it would be feasable in the DO world where most programs don't fill. Hmmm....

There is no such thing as an EM fellowship, despite what you may hear from some people on SDN. The American Board of Emergency Medicine only recognizes residency programs and will only grant certification via this route (the "grandfather" method was done away with a long time ago). Further, MD's cannot join DO residencies. The only way to become an emergency physician is to bite the bullet and go through a second residency.

(Florida recognizes the certification from the American Association of Physician Specialists, but it's the only state that I'm aware of.)

The next best thing if you love treating really sick people and procedres is to just to a 2yr. Critical Care fellowship. I know a lot of EM docs love Critical Care.

Yes there are 1 year fellwoships in EM , and they are considered good as well . Check this link out http://www.aafp.org/fellowships/other.html

Agreed. You will not be able to take the ABEM boards after a "fellowship" of any sort, which means that you will not be able to practice in many ERs.

Getting a spot for a "second residency" will be difficult, as you will have exhausted all or part of the funding CMS gives to the hospital to train you. So the Emergency Department will not only bear the cost of your salary, etc. but also will not get any of the extra payment CMS provides to them to train residents.
