Yes but how can you get referrals? Hospitals have such a large referral system you will be drained of business.

Well, depends what specialty we are talking about. For PCPs, it doesn't matter as much. You can always try to compete with the big systems by providing more value to your patients or move out a bit further where there is less competition.

For outpatient specialists, it depends a lot on location and the local market. For my specialty (rheum), the market seems to have opened up recently and any tier 3 city or lower is wide open. You can theoretically squeeze into tier 2 with a lot of business savvy and hard work. Tier 1 is impossible for most docs regardless of specialty.
