So if I did EMT classes at a community college, but never received a grade for them, how do I report this in AMCAS? The college website says there is a hold on my account that led to me never receiving a grade for any of these classes, but allowed me to receive credit for them.
This technically makes my gpa from there a 0.0, which would obviously hurt my cumulative gpa immensely if this were factored into it.

So how does this work? Do I have to report it if there's no grade for these classes?

Yes, you still have to report it. I'd suggest calling the community college and getting it straightened out.

I also took an EMT class at CSULB but the program itself didn't offer academic credits or letter grades unless you paid extra for it, which I didn't. Do I have to report anything at all?

Got an A in mine. It raised my GPA. Yes, you have to report it (officially).

Yes, as assuming you registered for the class, you gave enough personal information for the Student Clearinghouse to know that you attempted coursework there. So you'll need to send in an original transcript, which will say that no grade or credit was given. But you still have to report it.

I took an EMT class at my university. B+. It counts towards the GPA. Superjelly of @Rik1111 and his fancy A lol
