My ORB says that my REFRAD date is 7/1, but it also says that I've started AD payback on 7/21. HRC and separations are useless/don't answer emails. S1 seems to think that I finished my ADSO upon graduating medical school.

In my resignation letter for next year, I was going to pick July 1 as it is the REFRAD date. If HRC notices the 7/21 date, does it mean that I can't be REFRADed until the next "1st of" date (Aug 1st)?

Basically - can you have a mid-month separation date in the Army? I am concerned because I may not have enough leave if pushed into August for ETS

Back when I was in, my AFADBD was 21 Jan 98 and my EAS was 20 Jan 2003 . Following that logic your last day should be 20 July, but who knows what logic combat admin is using anymore.

I believe you will be out of the Army (or at least Regular Army) at the stroke of midnight on 22 July. Even though you fulfilled the terms of your contract twenty-four hours earlier, the Army tacks on an extra day because of...well, reasons. I have a few friends who have funky REFRAD dates, and they seem confident that they won't be kept around until the next month starts. I wish I could speak from personal experience, but my contract is fulfilled at the end of a month. You can request any REFRAD date you want on your memo, of course, and HRC and your transition office will change your orders as needed. By the way, for others reading, I think the services do this differently, so I don't think Navy or Air Force examples will help the OP much.

My concern is about missing fellowship start if I get pushed into August. I heard the navy did the"first of the month" thing and became worried. I also had a couple of friends get away with July 1st ets despite showing up about the same time I did

Yeah, the Navy is different and less friendly about this particular issue.

If your friends were coming onto active duty mid-month and separating less than X years later, then they definitely got away with one. If you were already on active duty from residency, then when you signed into is immaterial. I mention this because I got some bad information about this several years ago. To wit, I was told that the clock on your ADSO doesn't start until you sign in, which isn't true, assuming that you're on leave between residency and reporting.

I was a .civ resident, so my orders were "show up July 1-30"

As for the post above, I project 46 leave days between now and ets time. I need about 6 of those for sure before I can go terminal.

TDY for house hunting? That's something they used to do back in the day also. Basically a free week.
