A program ask to list elective experiences on CV. I provide a brief description of the elective, and then list a couple comments from the elective evaluation. For example;

Endocrine Surgery
Dr. Blah Blah
2 weeks on the endocrine service, included general surgery call.
Comments from evaluation;
"Excellent medical student"
"Working well above his level"
"Will make a strong resident"

Would this be considered tacky, or poor form? Thanks for your advice in advance.

Not sure about your school but all the comments on my evals in medical school were included on my dean's letter. Literally cut and pasted into it. Beyond that, yes its pretty tacky. Also places have no way of confirming that those comments are real. Also, everything you've listed is pretty generic and doesn't tell anyone much about you.

super tacky. as mentioned, any evaluation comments are included in your MSPE.

My MSPE it didn't include comments from my 4th year electives, just my 3rd year courses, so it didn't include any of those good comments I had (but the electives I cared above, the people who evaluated me also wrote me letters of recommendation so I was covered). If a program asks for it, i am assuming its on the CV you provide them and not on the ERAS application (since that is past due) then I think what you described is appropriate... you may want to (if possible) get a copy of the evaluations to have if they want to "confirm" anything you put from the comments. If they didnt ask for it, then yeah, would be tacky, but you gotta do what you gotta do.

a CV is just a list of achievements. it is not a resume. a lot of medical schools have you put together a resume but call it a CV.

google some professor's CV. you will see it has no info about the work performed. just job title and where.
