Examples of "bad" implants?

I'm studying for orals and wondering if anyone has come across examples of "bad" T&O implants anywhere online? Please let me know. Thanks!

#1 - Ovoids low in relation to flange
#2 - Packing superior to ovoids
#3 - Applicator rotated - need to repack
#4 - Tandem too posterior + packing superior to ovoids
#5 - No posterior packing (high rectal dose!) + applicator rotated

#6 - Tandem fell out of Smit sleeve, ovoids too low

Given this is 2016, I would also familiarize yourself with what T&O and T&R look like on CT cuts (for HDR). Same potential problems: ring not flush with cervix, tandem not bisecting, etc.
