Hello, if you are in the US or Canada, you are quite definitely not following your institutional policy for surveys. Please take this down, and then redo it to what your institution requires for notification. If you are from elsewhere, you need to read Dillman Mail and Internet Surveys for the usual protocols for how you send surveys out which includes identifying yourself and your institution.
