As I finish my final year of residency, I am soon to begin the likely ardous process of licensure. As I do have potentially derogatory information from my previous residency from a different specialty, I know that an explanation is in order.
Concerning that: what type of information needs to be in that explanation. Is it a narrative of events? What I have learned? A personal statement type document? How long is this? 1-2 paragraphs or 1-2 pages?

Imagine that you're on trial. Answer the questions posed to you honestly and as briefly as possible. Don't give extraneous information.

Don't try to explain yourself. You will fail.

If you had trouble in one residency, but then were fine in a second, the board is unlikely to make a big deal of it. Presumably, whatever problems you had (or were attributed to you) are solved, since you now have done fine. The key exceptions would be substance abuse, sexual misconduct (i.e. with a patient), billing fraud, patient privacy violations, etc. Those types of things may trigger closer scrutiny -- you'll still get a license, but perhaps with some sort of oversight / monitoring.

Agree with the above. Keep it short and sweet. If they want more detail, they will ask. It is most important that whatever you say matches whatever your prior program says.
