i was wondering if anyone could give me some insight into external rotations during 4th year. how difficult/competitive is it to get a placement in a specific city you are interested in? do some of them include room for free?

Each school does their selection a bit different. Some lottery and some do it based on grades. If that interests you, be sure to ask about it when you interview.
At PUCO we lottery for our sites and you do one selection at a time. So it is possible that while you are lottery-ing for your first site, someone else might take your second choice. So it is good to have a TON of backups 🙂
As for rooming - most of them do not include housing but some of them do. Again it depends on the site. Some will have free housing (like a few VA's, military sites, IHS sites) while others will having housing provided for a certain price. Even others will leave you all on your own to find housing - and in this case you can talk with the students who went there previous years and they may have suggestions. Also - just because some VAs, military sites, and IHS sites have housing, doesn't mean they all do.

PUCO is where I plan to go 🙂 This is probably a *****ic question, but with a lottery do you mean that they randomly pick students' names and if your name is picked first you get your first choice? Sorry, I'm sure I'm just having a brain fart, but I'm a little confused about how someone could get my second choice while I'm lotterying for my first if you do one selection at a time...wouldn't my second choice be available during the 2nd selection process?
Not always true. Yes Pacific does a lottery system and I personally know a few other schools that do that. However not all schools do that. I know that the UHCO externs at my current rotation do it based off a ranking system. There are pros and cons to each system. But personally, after hearing how other schools perform the rotation-selection process, I am very satisfied with the way PUCO does things.
