Externships; are they really that important?

I know this question probably has been asked before, but how much do externships matter?

I dont go to a big name program, but plan on applying to some for residency.
My grades and Step 1 are average.

I can get one 4 wk externship in before interview season begins next year. So should I try to go to a big name neuro program or save the money?

tough question. In most places, an externship sort of guarantees you an interview. Some call it a courtesy interview, in that they really wouldn't have invited you because you may not be really competitive. But you can never tell if you trully are a courtesy interview.

If there is a program you are set on, I say might as well check it out. I think an externship gives you a chance not only to shine, but to really see if the program is even worth applying to.

I know quite a few people who did externships at certain programs and didn't get invited for an interview. Lots of places don't do courtesy interviews, so don't count on an automatic interview if the rest of your application is lackluster. However, externships do show your interest in the field, which is very important to your application. And remember, a great letter of rec is the real prize!

Don't count on a courtesy interview. However, do meet with the program director at least once towards the end of the externship. During that meeting if the PD doesn't offer an interview, you should ask the PD if it's possible to arrange an interview before you leave...and if that is not possible, see if you can come back for an interview. Nothing wrong with promoting yourself a little bit, especially after you've worked your butt off as an acting intern on their service for 4 weeks.
 
smaller programs have 3 slots a year. While not generally that competitive they might have 1-2 students staying on or have a slot or two given away through connections, so some years something like an externship can be crucial to matching a specific program. Probably less of an issue for larger programs, but it does help show interest. Not crucial to match somewhere, but can be very important in matching to specific programs some years. It's hard to predict. If you audition well, I'd recommend it.
