Any thoughts on going into ERAS for the programs you are going to rank and assigning an extra (5th) LOR in place of one of the old ones? I figure programs might print and download it if they wanted to but if not it couldn't hurt right?

Probably wouldn't but:

1) most programs have already finished interviewing and their ROL so your letter will not be seen or change their ROL
2) if they do download it and see it, it might irritate them since most programs limit lettters to a max of 3 or 4

If you have a faculty member supporting you, at this late date, they need to CALL, rather than send a letter which may or may not even be seen. Waste of time, IMHO.

i think a 5th letter would be annoying. what does it say about you that your first four letters don't say?
