Hello,

I know in general Child fellowship is the only psych fellowship that you're able to do fast track. But I've heard of some fast track options other fellowships and was wondering if any has heard of fast track substance abuse fellowships. Has anyone head of those?

thanks

I believe this was talked about, but I don't think it ever happened. As of now, you have to have 48 months of general adult training to move on to a fellowship except child of course.

technically you can fasttrack into any fellowship if they accept you but you would not be able to gain board certification in that subspecialty unless you completed the full 4 year psychiatry residency (with the exception of child psychiatry). Non--ACGME fellowships (e.g. research, public psychiatry, psychopharmacology etc) you could do as a 4th yr.

This is an interesting idea. I have some follow up questions. So lets say I apply as a PGY-3 and they accept me into an addiction fellowship. After that I wanted to do a child fellowship. After the child fellowship could I be board certified in child psychiatry? Would I still need PGY-4 general psych to be board certified in addiction?

If I don't end up doing the child fellowship and only did the addiction fellowship after PGY-3 (or the non ACGME fellowship), would I still need to return to PGY-4 after the addiction fellowship (or non ACGME fellowship) in order to practice as a general adult psychiatrist?

Addiction fellowships are ACGME so you can't do one after a PGY-III. You could do any non-ACGME fellowship and call it your PGY-IV year because you are not getting ACGME credit twice for the same time. You could get stuck with the need for 12 months of continuous outpatient rule however.

Loyola addiction medicine (not adddiction psychiatry) fellowship was open as a pgy4 psychiatry position in the past though their website does not say that anymore.

MUSC is offering a 4th year addiction fellowship:

Medical University of South Carolina- Addiction Psychiatry
The Addiction Psychiatry Fellowship at MUSC provides psychiatrists with advanced training in the diagnosis and treatment of addictive and co-occurring psychiatric disorders, under the direction of internationally recognized experts in both the clinical and research areas of the field.

Prerequisites: Applicants must have completed 3 years of general psychiatry residency and passed USMLE 3 prior to entering the program. We sponsor J-1 or H1-B visas.

Wait so let’s say I did fast track psych into child and then wanted to do addiction (or forensics let’s say)
3 + 2 + 1

Would I not be board eligible for the non child fellowship because I did three years of gen psych?

Any other fellowships other than the addiction one at MUSC that you can apply to for your PGY-4 year?

Y0u guys are missing the point. As long as you meet all of the requirements by the end of 36 months of training, you can enter any training for the last 12 months to meet the 48 months of training requirement. It can be psychiatric tiddlywinks for all anyone cares, but you can not go into something that counts towards an ACGME approved fellowship and double dip getting both a PGY-IV year and a PGY-V year in one year. Child and Adolescent fellowship excluded because it is 2 years. You can go into a CA fellowship and quit after one year and be general psychiatry board eligible. Your fellowship being your PGY-IV time. ABPN is trying to get away from the "board eligible" language by the way.

No, I get it. But when I look up these nonACGME accredited fellowships, almost all I've found mention being board eligible before applying. I'm wondering what my options are if I want to do a non ACGME fellowship in my Pgy4 year

MUSC Addiction psychiatry fellowship "how to apply" as seen on their website

"You must have completed an Accreditation Council for Graduate Medical Education (ACGME) accredited psychiatry residency prior to entering the program. Training in addiction psychiatry that occurred during the general residency training will not be credited toward the 1-year requirement."

Oh I see, so maybe they let PGY-3s apply and do the fellowship but that doesn't mean you can actually practice it as an addiction psychiatrist.

No--it means that you don't get Fellowship credit for addiction rotations done before completing your four full years of General Psychiatry residency; and that they expect that you've completed three years in *Psychiatry*, so if you began as a PGY2 transfer from a TY or other specialty program, you would have three years of psychiatry training to qualify to start.

What about this the scenario of having completed a medicine TY for neurology then transferring into a psych program where you did not repeat medicine months the first year, because they made you “restart” PGY level as a PGY”1”? By the end of the PGY3 year (4th year of residency) all acgme requirements are accounted for including the 48 month rule. Per ABPN this is acceptable. My question is what about graduation? Could you take the board if PD allows and go to fellowship (especially if it’s non-ACGME like neuropsychiatry)?
