Fasting intermittently

We learned about its benefits during our nutrition class. Caloric restriction has started to fall by the wayside especially after trials in primates failed to show a benefit. There has been interesting small scale stuff published surrounding reduction in oxadative stress in mitochondria around intermittent fasting. IIRC, the evidence for large scale is not a slam dunk, like any nutritional study, however the results are interesting enough and worth a shot for most people willing to give it a try.

Anecdotally from the guys at the gym/reading the weightlifting forums (so, bro-science), it works if you do it correctly, i.e. fasted time only counts if you haven't eaten anything in the past few hours and are awake. This typically means eating all your calories within a three hour window or so. Doubt many patients are willing to do this.

For example, my buddy has gotten very cut doing this diet, but he wakes up at 6 am, does his inpatient shift, and eats his first (small) meal at 4 pm, lifts at 5, then eats dinner at 7 pm and is done for the day.

The widely-publicized diet involves just skipping breakfast, which is known as the obesity diet.

Anyone know if this stuff is real science? I’ve been getting asked by a lot of patients about intermittent fasting but haven’t really heard of it before..any thoughts on this?

There is evidence that IF works. Good evidence. It's all related to body fat set point directed at the brainstem metabolism level by insulin and insulin resistance. IF looks to begin the reset of bodyfat set point by decreasing the time under insulin load. All other "diets" fail. Eventually - the reason being inability to reset the bodtfat set point because of too much insulin and relative insulin insensitivity. The metabolism driven by the brainstem centers is a mother****er. And it will eventually slow down your metabolism enough that even on a lower calorie diet with exercise you will gain weight (fat). You don't "lose muscle" by not eating every two hours like someone is bound to chime in and say. The body stores fat to burn it and it will if you let it. But you need to let it. So don't eat so often.

That's the basics of IF. Suggest they start by skipping breakfast. Which isn't "the most important meal of the day" and doesn't "kickstart the metabolism". Find THAT data.
