Hi,

If you have already left or withdrawn from a fellowship and are applying for another fellowship ,is it necessary to let the recent fellowship programs know that you had left the previous one or not?

Technically no, but you're going to get the question 'what were you doing with yourself between x and y?' Probably better to be up front with it. I know a number of people who have switched fellowships (from one field to another, not program to program), and they ended up fine. Make sure you spin it in best a light as possible, and explain how this time you're sure this is what you want to do.
