Is it necessary to do an industry fellowship to get a job in a pharmaceutical company? In something like medical affairs, drug safety, or medical communications?

Probably not 100% necessary, but pretty close to it.
 
You need to do a fellowship OR residency. residency bc you would work in a hospital a few yrs and develop a "clinical speciality" (like cardio for example) and then be hired to work in the cardio drug dept at a pharma company.

the only ppl I heard of who got hired without fellowship or residency either interned for pharma like every single summer in pharmacy school OR are super old and fellowship/residency wasn't big then OR have some master's degree in a specialty field
 
I have seen pharmD's land a pharma job without completing a fellowship or residency. They are usually people that completed at least 1 internship or rotation to get that "foot in the door" and to network. Entry level positions in pharma are usually drug safety or medical information for those without postdoc training (fellowship/residency). It is not easy, but it is not impossible...It will come down to who you know..
