fellowship without residency

i have done internal medicine residency in my country and also MRCP..is it possible to apply for a fellowship in u.s without repeating residency..thnx

It is possible, and 10 years or more ago it was pretty common. But as fellowship spots are getting more competitive it is much less common than it used to be. Your best shot is if you have a strong research background (like a PhD or a large number of first author papers) and are coming over to do a research-heavy fellowship. I'd start contacting PDs at programs you're interested in to see if they'd be interested in having you.

One downside is that you may be required to sit for USMLE (at least Steps 1 and 2) which will delay your application for at least a year.
 
- You will need USMLE steps 1,2 &3 as long as you are going to encounter patients directly.
- You can look for a non ACGME accreditated fellowship but downside is you can't practise in US after completion of same.
- Have known a few people who came and joined such a program eg. liver transplant fellowship and after completion joined surgery residency and completed the whole residency (sort of doing it backwards and all over again!!!)
