How tough are fellowships in neuro to obtain? If I have my hopes up for something like neurophysiology or sleep is it possible I won't be able to secure one of these spots?

I believe that it is not a question of whether you can get a fellowship but where. Most people obtain a fellowship position, but the more competitive fellowship spots will typically take internal candidates from their own program. Therefore, being at a residency with good fellowship training may be an advantage in these situations.
