Filling out LOR section on AMCAS

What do I put for primary contact/author title? Do I put as it is listed on the school website?

For example my LOR info is listed below:

Professor A:
-Organic Chemistry Prof. Ph.D. On school website it says "Instructor"
-Do I put "Ph.D Instructor" or just "Instructor" or just "Professor"

Professor B:
-English Prof. Not Ph.D. On school website it says "Visiting Instructor"
-Do I put "Professor" or just "Visiting Instructor"

Professor C:
-Bio prof. Ph.D. On school website it says "Associate Professor"
-Do I put "Ph.D Associate Professor" or just "Associate Professor"

Professor D:
-Religions prof. Ph.D. On school website it says "Visiting Instructor and Undergraduate Director"

Professor E:
-GenChem prof.. Ph.D. On school website it says "Faculty"
-Do I put "Ph.D Faculty" or just "Faculty" or just "Professor"

Supervisor at Non-Profit:
-"Match Support Specialist" is her title.


I know these questions are obvious but I just want to make sure:
-For letter title, can I just put the name of the letter writer? Like "Dr. A"
-For the "email" and "phone number" at the bottom, do I put the contact info for the "organization name" or the actual letter writer?

Just put professor; in the end it wont matter
Thanks @gonnif Is it okay to ask my professor who wrote my letter if I can see the letter they wrote for me? I saw your post in another thread saying this is okay. I was wondering if AMCAS explicitly says this is okay on their terms so I could reference it when asking my professor if they could send me a copy of the letter they wrote for me.
 
You have no right to demand to see the letter
The professor has all the rights to show it to you
You can ask, but be very nice about it as many profs are hesitant to show it to you
Ok is there something I can reference for them saying that it is legal for them to show me? So they feel more comfortable about showing me.

When I email them I was planning to say something along the lines of:

"Thank you again professor x for your letter. I was wondering if I could have a copy of the letter for my records. The medical school application service states that this it is acceptable for letter writer to share their letter with the applicant according to this link ____, however, I understand if you do not feel comfortable sharing the letter with me. I appreciate your time."

Background: Letters of evaluation (often recommendations) are an important part of each medical school application. Admissions officers and staff, admissions committee members, and pre-medical advisors have a mutual responsibility in keeping the information conveyed in letters confidential, unless explicitly indicated otherwise by the letter’s writer. A confidential letter signals to the reader the letter’s writer expectation that his/her comments will be shared only with the audience for whom they are intended and not shared with the applicant or anyone else for whom the information is not intended
Thank you @gonnif! Seems pretty strict, I think referencing this link might steer my letter writer away from sharing the letter with me. Maybe I just wont ask...
