I was hoping some of the attendings out there could give me some advice. I'm currently in the process of looking for my first job, and have been sending my CV out to a bunch of groups. The places I'm applying to each has a different type of setup in terms of how you will practice. Assuming salary and benefits are comparable, would you have any hesitation joining a group straight out of training that does all supervision (4 CRNA rooms, including CRNA hearts) versus a group where you do your own cases. I guess what I'm trying to ask is can you still continue to develop and refine the skills needed to be a great anesthesiologist if the only thing you do is supervise all day and not do your own cases?

I wouldn't recommend a 100% supervision job straight out of residency. Ideally you would spend a few years on your own, honing your skills and speed, as well as your sixth sense, and how to manage complications and emergencies on your own. I did, and it was very beneficial. However it's not a realistic option for everyone.

The only way to grow is by doing cases. You could do 4 times the cases when you supervise compared to solo. Granted, you will be liable for mistakes you didn't make.

isn't it 8x at its max?

The liability is a lot greater when suprevising four rooms.. And supervising is NOT EASY. It requires a fair amount of time management skills, understanding the skills of those around you as well. DOing cases on your own and sitting on the stool is the greatest job known to man. You have control over everything.

This guy knows the deal-io! 👍

The ideal position from a skill set perspective would have a mixture of MD administered anesthesia and supervision. Diverse case mix is a given.
If you do choose to take a position with a lot of supervision, be aware that there is a much different dynamic that exists when the CRNAs are employees of the anesthesiologists as opposed to the hospital.
Odds are that the position supervising 3-4 rooms including hearts pays better per hour of time worked than most MD administered gigs. Odds are that the job doing your own cases has better mental health benefits.
Your question stated that salary and benefits are comparable. Assuming that, and that the time commitment is comparable, I would jump at the job doing all my own cases.

No truer words have ever been spoken. CAUTION: Once you get a taste of the scam of "supervision" however, you will never want to do your own cases again.
