I think that's more a term to denote the "main medical center (hospital)" vs its CBOCs and/or relatively small VA hospitals that dont have the full range of medical services, no? Like the difference between Nashville VA and Murfreesboro VA, for example?

Generally larger VA hospitals with a very wide range of specialty services, as well as lots of ongoing research. Examples would be Houston, Seattle, Minneapolis, San Fran, Boston, to name a few. The VA has an in-house designation that uses flagship monikers, but it doesn't mean much. For example, Tomah is included on that list, which I would not consider a comprehensive site, especially in the company of the larger VA hospitals.

Yeah, I was also thinking VAs with MIRECCs or Centers of Excellence.
