I am a transition intern in a 4-year anesthesiology program. I'm scheduled to do a MICU month next month. However, the chief resident of internal medicine offered me to do a floor month, instead of a MICU , due to scheduling conflicts. If I accept this offer, I won't end up doing a MICU (or SICU) month during my transitional year. What is the policy in anesthesiology residencies in regards to ICU months during intern year? Is an ICU month required?

I don't know that there is a policy on it. You have an ICU requirement for your residency, but internship is separate. You will probably get more out of and like better your ICU time. More ICU time is better preparation for anesthesia and in-training exams. It is closer to anesthesia than floor medicine is. I personally would prefer the ICU. With that said, since you're in a 4 year program, you may decide to not rock the boat and just do it. You'll get the ICU time eventually. Consider asking them for something else, especially if you were looking forward to the time. Or tell your PD about the situation and ask for their opinion. I was in a 4 year program as well and had some schedule changes because of departmental and work force issues. But my PD was in charge of it, not the IM chief.

Doctah' Lee,

You are required to do a total of 2 months of critical care training in your CA years. Just ask the PD if they integrated one of those months into your transitional/CBY. If it isn't integrated they you have nothing to worry about.

As far as CBY year goes you do NOT need to have a unit month. Floor bloooooooooows and I'd rather hit the unit. You'll actually learn something vs being a full on scutmonkey. On the other had I know its warming up, there is fun to be had, and the MICU can be grueling.

Hey Jason,

I'm torn between a floor month vs ICU. I spoke with Shingo and Derek Lambert who think I should accept the offer to do a floor month instead. Their reasoning is that it's just not worth dealing with Dr. Liao's prick attitude. Did you really learn in the MICU? I think I'm going to try to sneak a floor month without my program knowing.
