What does it take for a FMG to get into ortho residency? Besides top usmle scores?

How about:

Good letters/references/contacts?
Maybe some good research or publications?
Maybe a good Foreign med program?

It is of course luck of the draw. Kinda the same thing for U.S. grads but maybe a little more of it. Look for programs that have a track record of being open to accepting FMG's. You could always contact residents at the programs you are interested in and ask them?

rare for fmgs to get into ortho but some do
most are from caribbean or european med schools with extremely high usmle scores

There is an FMG (SGU) ortho resident @ UMDNJ Newark. He Had ridiculous board scores and amazing LOR's. Its hard but not impossible.
I have also seen some other FMG senior residents (PGY5/6) from India. These guys were already practicing orthopods before coming to the US. I dont know how their boards were... but these guys know their shiznit cos they are treated like attendings.
 
I think its possible but difficult.

The reason is is that so many US graduates aspire toward ortho that there might not be many residency positions left over for an FMG.

In my unlearned opinion a FMG shooting for ortho should:

1. Aim for community programs
2. Do away rotations at these programs and impress the PD and residents
3. Publish in ortho
4. Get good letters from known orthopods
5. Have excellent Step 1 scores (>240)

If you do all this, you should have a chance.
